Get Your Achilles Rehab Working: The Loading(Strengthening) Threshold Your Tendon Is Waiting For
If you've been doing your calf raises every day, being consistent, "doing the work" and your Achilles pain still hasn't budged, you're not imagining it. It's one of the most common frustrations we hear from runners and walkers coming through our clinic, whether they're hitting the local parkrun, getting ready for marathon season, or just trying to get through a normal work day without heel pain.
Here's what's actually going on: your tendon isn't changing with your effort. It's waiting for you to cross a specific line.
Achilles Tendinopathy Is Common, Here's Why
Achilles tendinopathy is an overuse injury affecting the thick tendon connecting your calf muscles to your heel bone. It develops when the load going through the tendon, from running, walking, hill work, or sudden increases in activity, outpaces the tendon's ability to recover and adapt (1). Locally, we see it a lot in runners building up for events, weekend warriors returning to sport, and people who've recently ramped up walking or hiking miles in preparation for a Great Walk “aka” you are asking too much of your tendon!
Typical signs include:
- Pain or stiffness at the back of the heel or mid-calf, often worse first thing in the morning
- Pain that eases as you warm up, then returns after activity
- Swelling or tenderness along the tendon
- A "creaky" or grinding feeling when you move your ankle
If any of this sounds familiar, it's worth getting it properly assessed rather than pushing through. Early, correctly loaded rehab tends to recover faster than pain that's been managed passively for months.
Why "Doing Something" Isn't the Same as "Doing Enough"
This is the part most people get wrong, often with good intentions. Tendons don't respond to activity in a straight line, where a little bit of loading gives a little bit of benefit. They respond more like a switch: below a certain strain level, your tendon treats the load as no different to daily life, walking to the letterbox, standing at work, general movement, and does nothing to adapt (2). Only once the load crosses a specific threshold does the tendon start the remodelling process that actually builds capacity and reduces pain.
Research on tendon loading backs this up clearly. Studies comparing different training intensities on the Achilles have found that light, low-strain loading produced no measurable change in tendon stiffness or structure. It was only once loading reached a higher strain magnitude that researchers saw genuine adaptation: increased stiffness, improved tendon structure, and better tolerance to load. Doing more repetitions at a low intensity didn't make up for missing that threshold (3).
That's why gentle stretching or very light exercises alone often plateau. They can feel productive without ever giving the tendon a strong enough signal to change.
What This Means for Your Rehab
In practice, effective Achilles tendinopathy rehab usually needs to:
- Start where your tendon can tolerate it - early-stage rehab often begins with isometric holds to settle pain and build a baseline.
- Progress to genuinely heavy loading - slow, controlled resistance work (like weighted calf raises) that's demanding enough to trigger tendon remodelling, not just "felt".
- Build in enough volume and consistency - sporadic effort at the right intensity still won't be enough; the tendon needs repeated, structured exposure over weeks.
- Respect a pain-monitoring approach - loading should be firm but controlled, guided by how the tendon responds over the following 24 hours, not pushed through sharp pain.
This is also why generic online exercise sheets often disappoint people. Without an assessment of your specific loading capacity, stage of tendinopathy, and training goals, it's easy to under-dose the exercise and stay stuck in “no man's land”, working hard enough to feel tired, but not hard enough to actually change the tissue.
Common Mistakes That Keep Achilles Pain Hanging Around
We see a handful of patterns repeatedly in people who've been struggling with Achilles pain for a long time:
- Resting completely and hoping it settles. Total rest can calm symptoms short-term, but tendons need progressive load to actually remodel. Without it, pain tends to return the moment you resume activity.
- Stretching instead of strengthening. Calf stretches feel like "doing something," but stretching alone doesn't provide the strain needed to trigger tendon adaptation.
- Stopping as soon as it feels better. Symptoms often settle before the tendon has actually rebuilt capacity, so activity is ramped back up too soon and pain flares again.
- Ignoring footwear and training surface. A sudden switch to more hill work, harder surfaces, or worn-out shoes can quietly increase Achilles load without you noticing.
- Self-diagnosing off Dr Google. Achilles pain can sometimes be confused with other issues (like tenosynovitis or retrocalcaneal bursitis), which need a different approach, an accurate diagnosis matters before you commit to a rehab plan.
